Please completed a fish registration form.

 Bill’s Musky Club must receive the official fish registration form(s) no later than 30 days after the catch. Emailed or mailed forms are accepted.  All areas marked with a * on this form must be fully completed; failing to do so may result in a disqualification for any monthly/yearly awards.
